Question about MTF transitioning in the workplace

Someone who is beginning a gender transition process at their workplace asked me today what points are usually discussed when meeting with HR to discuss the transition, in terms of the various ways in which the company should be prepared to support the person transitioning. Anyone got any good website pointers to share? Thanks!
